PATIENT NAME: Alice Liddell PATIENT ID: AL1865 DOB: May 4, ADMISSION DATE: March 15,

PRESENTING SYMPTOMS:

  • Vivid hallucinations of a fantastical world she calls "Wonderland"
  • Delusions of talking animals and anthropomorphic playing cards
  • Severe dissociation from reality
  • Erratic behavior, including attempts to "shrink" or "grow" by consuming various substances
  • Episodes of uncontrollable laughter followed by periods of deep melancholy

BACKGROUND: Alice Liddell, a 22-year-old female, was admitted to The Carcosa Institute after her parents reported increasingly concerning behavior. Alice claims to have fallen down a rabbit hole into a world she calls "Wonderland." She describes encounters with various fantastical creatures and experiences that defy the laws of physics and logic.

INITIAL ASSESSMENT: Patient presents as a well-spoken, imaginative young woman with a tendency to slip into elaborate fantasies. She shows signs of disorganized thinking and difficulty distinguishing between reality and her hallucinations. Alice maintains that her experiences in "Wonderland" are real, despite all evidence to the contrary.

DIAGNOSIS:

  • Primary: Schizophrenia with prominent positive symptoms
  • Secondary: Possible dissociative identity disorder
  • Differential: Temporal lobe epilepsy, drug-induced psychosis (to be ruled out)

TREATMENT PLAN:

  1. Antipsychotic medication: Risperidone 0.5mg BID, to be adjusted as needed
  2. Individual therapy sessions 3x weekly focusing on reality testing and coping strategies
  3. Group therapy 2x weekly for peer support and social skills development
  4. Art therapy to provide a constructive outlet for her vivid imagination

To be placed in the care of Institute Chief Psychologist Dr. King, until deemed fit for release
